A pre-booked consultation is held to establish care needs (via phone/Zoom/Teams or a home visit).

Who's it for:

  • For those beginning to explore the care and support sector and where relatives live far away or struggle for time.
  • This package is for you if your relative does not have to sell their home or the family does not have to relocate from the family home.

Consultation includes:

  • Understanding care needs, requirements and wishes.
  • Discussion around potential services (e.g. care home, domiciliary care, funding, social services, support).

What you get

  • A detailed report detailing the above which provides a holistic review of current circumstances, captures wishes, and identifies the need for support.
  • Desktop and telephone research to identify suitable services.
  • Reviewing the contract details and identifying any hidden costs.
  • • A shortlist of agencies/care homes that meet the search criteria, including:
  • • Service offered
    • Inspection reports, reviews and ratings
    • Costs
    • Availability
  • Information on funding.
  • Information/checklist on what to look for when assessing care providers.
  • A follow-up call to go through the report and answer any questions.
  • Arranged accompanied visits to the family home (for chosen agencies) or the chosen care home.
  • A meeting to discuss thoughts (either in person or remotely).

Once the preferred service has been chosen, support with:

  • Booking the service/care provider.
  • Putting into place procedures to ensure a smooth transition from home/hospital to the care home.
  • A relocation checklist for the family.
  • Arranging further visits to the care home and, if required, arranging transitional stays at the home.
  • Calls with the client and family to ensure a smooth transition.
  • If required, attendance on the day of the move to provide support.
  • Two months of follow-up calls to deal with any concerns or questions (written summaries can be provided on request).
